You will receive separate bills for the facility, surgeon and anesthesiologist.

Bring your insurance card and social security numbers to Salem Surgery Center. Pre-approval must be confirmed prior to surgery. It is your responsibility that all insurance authorizations are done. Your surgeon's office will assist you with the pre-approval and other related questions you may have.

If your insurance does not cover the procedure, or you do not have insurance coverage, please contact the facility before the day of surgery to discuss financial arrangements.
